Team of professionals ensures ongoing communication with physician, case managers,
the insurance company, family and provide an interdisciplinary approach to patients.

Whether you are recovering from surgery or illness, or living with a chronic disease like diabetes or COPD, Prime Home Health Services can make hospital visits shorter and less frequent with home-based skilled nursing, rehabilitation, and chronic disease management. We work closely with your physician to coordinate all aspects of your care, focusing on helping you recover and regain your independence.

We understand that planning for the care of each patient is a process that requires compassion and professional expertise. Recognizing that our patients are understandably ill, sometimes anxious, and under stress, our trained staff are the best health care professionals to offer care, comfort and meet each patient’s specific care needs.

Skilled nursing is provided by a Registered Nurse or Licensed Practical Nurse and includes coordinating the plan of care your physician has prescribed, monitoring vital signs, giving injections, and providing wound care, among other things.

Home health aides don't have nursing licenses, but they support your nurse by providing assistance with bathing, dressing and other personal care.

A doctor will create a plan of care based on a patient's specific needs. Highly trained and certified physical, occupational and speech therapists help patients meet their goals and live more independent and productive lives.

Our physical therapists will visit you in your home to help you perform daily activities. The goal of physical therapy is to help you regain movement and strength.

Occupational therapy is designed to help you regain your independence by performing daily activities by yourself. Occupational therapy is sometimes ordered by a doctor without other skilled care.

Speech-Language Pathologists (SLP) specialize in the evaluation and treatment of swallowing, cognition/thinking skills and communication disorders to help your regain independence and improve your quality of life. After a stroke or accident, speech therapy can help you recover your swallowing and communication skills.

We know physical issues aren't your only concern when recovering from or managing an illness. Medical Social Workers visit your home to help you with the social and emotional concerns that may accompany your health care issues.
